INSTRUCTIONS:
Complete document on 1 page. Use 14 inch paper if necessary for originals.

Execute 3 at Notary. One original to Secretary of State. Others may be down sided as photo copied.

Gather in one file:

A. Birth Certificate. Use exact language and all identified files or reference numbers on document inserted in Identity document
B. Social Security Card. You must have front and back numbers. Photo copy both sides on a single page. Staple to Identity document. Sign copy with thumb print over signature. Insert the following language in your own hand writing.
Accepted, ratification of gift and recognition of lawful State Citizen. ( In the case of citizenship by immigration, insert, after State Citizen, by proper immigration contract, … date of citizenship, and passport identifier. )

Insert information as indicated.

Insert passport picture on left side and signature with reservation of Rights below, on right side. Place right thumb print over the signature. Any color ink.

Have Notary Seal affixed. Make sure you identify the title of the document in the Notaries book prior to signing.
